Note: Membership fees are paid annually. An additional fee of up to $75 per person, per year will apply for claims, balance bills and hospital confinement audits where no claim dollars are recoverable because all charges have been processed and paid correctly by the insurance carrier; or a 10% fee (minimum $50, maximum $2,000 per person, per year) will apply on any amounts recovered from claims, balance bills and negotiated payments*. Please discuss potential fees with your Patient Advocate and a signed cost estimate and consent form will be required before work commences. *10% amount will be based on reduced Explanation of Benefit amounts from insurance carriers and billing invoices reduced by our efforts, including negotiated payments** with service providers or hospitals / **Requests for bill reductions due to Club Member inability to pay, will be referred to legal counsel.  Legal counsel will individually review and price on a case by case basis.
